We are seeking a Home Care Intake Program Assistant to provide administrative support to the Home Care department's Intake Team and perform related functions as assigned by the program management.
What you will do:
Assist with all aspects of bringing new consumers into the HC department Collaborate with the program management regarding information related to file set up, issues with completed paperwork, and quality of referrals being submitted.
Review and correct data for new consumers within record system.
Provide accurate reports as needed.
Assist staff in sending and submitting Waiver paperwork for consumers.
Provide phone coverage as needed What you will bring:

High School diploma or equivalent; Associates degree preferred Proficiency with Microsoft Office Suite or related software Minimum one year of relevant experience.
